Order Form

CRPA, Inc. offers a unique memorial program to honor and remember your friends and family.

The Memorial Tree Program allows an individual to donate a tree which is planted in the Cherokee Recreation and Parks Agency park of your choosing. The tree's location within the park will be at the discretion of the Landscape Coordinator (*trees will not be planted from May-August and there will be a one-year replacement guarantee on all trees).

The tree donations are designed to honor the birth of a child or to express sympathy in the case of the death of a friend or relative.

For each tree planted, a certificate representing the occasion is mailed to notify the recipient family of the gift. Trees range from $45-$115.

If you are interested in donating a tree, the descriptions are below. The application can be downloaded on this page and should be faxed or mailed to Neely Motiejunas at (770) 924-7890 or 7545 Main Street, Building 200, Woodstock, GA. 30188. All donations are tax-deductible.

Memorial Tree Information
  • Trees will not be planted during the months of May – August.
  • There will be a one-year replacement guarantee on all trees. No replacements will be made after one year.
  • Placement of the tree will be at the discretion of the Landscape Coordinator within the park of their choice.
  • Trees will not be designated with any type of sign or plaque.
